Mic Booming Techniques

At first thought, booming seems like a pretty straight forward process but it's actually one of those things that takes a lot of finesse and technique in order to master capturing a clean dialog track. I was wondering if anyone has had any unique experiences on set that took some clever thinking to capture the moment. Maybe some other basic things could be shared as well. Like what mic would you normally recommend? How often do you choose to boom from underneath and why?
